MAPOLY UPDATE REGISTRATION PROCESS

1. Pay your acceptance fee(N25,500)
2. Print your acceptance receipt clearance form online
3. Visit the Health centre @d skul Campus to obtain your medical forms..
4. Visit d hospital and get ur medical examination done.. And ensure d medical forms are appropriately filled, signed and stamped where necessary..
5. Return the medical form back to the health centre along side with ur Xray. The form will then be signed and stamped @ d health centre..
6. Visit ur HOD and school officers in respect to ur various department schools to sign their own part of the clearance form.
7. Take the stamped to d admission office((small admin)) for the admission officer to sign his own part too...
Note: the following documents is needed and will be attached with the clearance form:
1. Original O'Level result
2. Birth certificate/ sworn affidavit
3. Original JAMB/UTME result (ND only)
4. ND statement of result (HND only)
5. oath form,
6. medical form
7. parent indemnity form (would be
provided by the school).
Note- Get 20 passport nd 4 photocopy of each document u collect nd stamp..
