only for FULL TIME student WHO DID NOT CHOOSE MAPOLY
in JAMB /UTME at all.

**** CHANGE OF COURSE : This form cost #10,000. It is for
student(FULL TIME AND EVENING) who wish to cross over
from the course they were offered to the one they intend to
do, also student who their O level result did not meet up
with the course they put in for.
NOTE : VISIT THE ADMISSION OFFICE TO VERIFY OR CONFIRM
YOU CAN CHANGE YOUR COURSE BEFORE PAYMENT.
PAYMENT IS NON REFUNDABLE.

****SECOND CHOICE : This form cost #10,000. It is ONLY for
FULL TIME STUDENTS who chose MAPOLY as a Second choice
in the JAMB /UTME

PLEASE NOTE : CHANGE OF INSTITUTION AND SECOND
CHOICE FORM DO NOT APPLY TO EVENING PROGRAMME OR
PART TIME PROGRAMME.

2. SIGNING OF CLEARANCE FORM BEGINS
The long awaited signing of clearance form has begun, the
following documents are to be presented for Clearance.
1. CLEARANCE FORM
2. ACCEPTANCE PAYMENT RECEIPT 3. O LEVEL RESULT
(certificate for WAEC 2013 downwards, statement of result
for WAEC /WAEC GCE /NECO 2014 till date)
4. ORIGINAL JAMB /UTME RESULT (ND ONLY)
5. CHANGE OF INSTITUTION /CHOICE FORM (ND ONLY)
6. ND STATEMENT OF RESULT (HND ONLY)
7. IT COMPLETION LETTER (HND ONLY)
Passport photograph must be attached to all documents.

3. MAPOLY RESUMES AS MANAGEMENT GIVES DEADLINE FOR
REGISTRATION.
The Management of Mapoly has announced December 14,
2015 as the official date for resumption for 2015/2016
academic session. All categories of students Except student
on SIWES (to resume February 5)expected to resume and
begin registration for the new session.
Also, the Management has announced February 22, as the
deadline for registration for the semester.
